well if you get a 403 - permission denied, I\'d think it could be a chmod issue, but all files in the dir are readable... 644...

Its fine if you refresh the page. Not sure whats causing it either, but I think its a server problem - possibly because the page loads itself a few times over.

It was user error... someone trying to load the page too many times in a second...

This is triggered when a user accesses a page more than 5 times in a second... they are blocked for 120 seconds.
This is what happened here. The server read the page being accessed too many times consecutively by the same user, and blocked them.
